They really didn't have much social contact before joining the Host Club, and the other members state that it took as long as Haruhi joining for them to fully begin to open up, so for at least a year they were incredibly socially stunted.
It's especially clear in the episode "The Twins Fight! This would be bad enough if it was just fueled by emotions, but it was actually a calculated prank on their part, showing a big disregard for the feelings of others.
Hikaru and Kaoru are described as always doing everything together, and while that is true, Kaoru begins to mature much more quickly once their inner circle opens. This is acknowledged pretty clearly as time goes on, and we're shown that although Hikaru is described as being the 'leader' twin, he's actually much more dependent on Kaoru than his twin is of him.
It's an incredibly interesting dynamic, and it's great to know that the act they put on at the club really is just an act. Kaoru wants Hikaru to be able to blossom into his own person, while Hikaru seems content to keep their world at just him, Kaoru, and Haruhi. It's fantastic seeing it played out with all the twists and complications that follow because of this. One thing that is definitely the same between them is their natural love of playing pranks.
Haruhi states in one episode that Hikaru seems just slightly more devilish than Kaoru, but this is still one aspect they're much more similar in.
The two are happily willing to carry out incredibly elaborate pranks at the expense of others' feelings, simply because they find doing so to be quite amusing. Especially in the manga that covers much more of their lives, the two grow out of this over time, but we'd like to bet they still pull harmless pranks every now and then. In vol. Being as this is yet another visual, most of what is said in the Manga section stands true, except a lot easier. This will also be the smallest section being as everything from the manga section applies here. Vocally, it is easy to hear.
In the Japanese audio at the beginning of the episode, the club visits Haruhi in the worst possible home condition. They enter her house and she serves them tea in chipped cups and measuring cups which Kaoru is drinking tea from while talking to his brother and Tamaki. This is a joke. As members of the Host Club, Kaoru and Hikaru are the "Little Devil" types, as they are mischievous but not malevolent troublemakers. They are famous for their "Forbidden Brotherly Love" package, carrying out twincestuous schticks for the entertainment of their female customers.
A favorite game of theirs is the "Which One Is Hikaru? Game," in which they cover their hair with matching caps and ask people to identify Hikaru. No one except for Haruhi has guessed correctly, with logical reasons for doing so. Due to this, and the fact that she sees them as unique individuals, both brothers develop romantic feelings for her. Kaoru and Hikaru are both wealthy and playful; however, Kaoru is the calmer and more mature of the two. When apart from his elder brother, Kaoru becomes a pleasant person, acting in a selfless and serious manner; but when he is with his elder twin, he reverts to being a scamp-like individual.
Though both of the Hitachiin brothers are co-dependent when it comes to each other, Hikaru is certainly much more dependent on his younger brother than the opposite.
In fact, it's Kaoru who's the first to realize that their co-dependency would have to end if both of them had any real desire to grow and change as their own people. Kaoru is also much more mature and level-headed than his twin, showing much more emotional maturity and being the reason why Hikaru can work through most of his problems.
In the anime, we only get to see Hikaru going out with Haruhi, but in the manga, Kaoru does as well- and it's actually established that both twins are in love with her, though only Kaoru is aware of both of their true feelings.
He ends up chasing after Hikaru after he runs off to try to avoid facing his own feelings, and is the reason why Hikaru is able to realize and take a chance on his own feelings for Haruhi in the first place.
Kaoru truly is sweet, deep-down, unlike the mischievous side he typically shows. While matching his twin when it comes to levels of hyper-activity and mischievousness, Kaoru is much more mature when left on his own, and it shows in the subjects that he gets the most enjoyment out of, being modern Japanese literature as well as English.
It perfectly suits his slightly more introspective worldview compared to his brother, making him seem like more of a match for someone intellectual like Haruhi. To add to the previous point of Kaoru being more mature when left on his own, it's like a completely different shift when he's apart from his brother.
On his own, Kaoru is shown to be calm, mature, and overall pleasant to be around, acting both serious and selfless.
